Outlined below are the steps and requirements for data privacy requests, including complaints, addressed to Milliman (“Data Privacy Requests”):

Consumer Report Requests: If your request is about a medical or prescription report (a “consumer report”) that was sent to an insurance company for an insurance application or claim, please contact the IntelliScript team:

Please include your full name, current mailing address, email address, phone number, and any report reference number (if you have one).

401(k), Pension, or Health and Wellness Plan Participants: If you are a 401(k), pension, or health and wellness administration plan participant, please contact the EBAC team:

Please provide your full name, email address, phone number, the last 4 digits of your Social Security Number, and date of birth.

All Other Requests (including complaints): For any other data privacy request, please contact the Milliman Data Privacy Request Team at: [email protected]. The team will provide additional instructions on the information required, depending on the type of request. California residents may also submit requests by calling 1-866-467-8688 + service code 740.

When submitting a Data Privacy Request to Milliman, you acknowledge and understand that the personal information you provide will be processed for the purpose of handling, verifying, and responding to your request. Requests, including complaints, sent to [email protected] are managed by Milliman’s Data Privacy Request Team based in France. To the extent necessary to determine whether relevant Milliman affiliates, entities, or service providers hold any information about you, your information may be accessed by or shared with them. Where the processing of your request, including the sharing of information, involves entities located in countries outside your place of residence, the associated international data transfers will be subject to appropriate safeguards in accordance with applicable data protection laws. By submitting your request, you acknowledge that such processing, including any required cross-border transfers, may be necessary to respond to your inquiry. Where consent is required under applicable law, submission of your request will be treated as your consent to such processing, to the extent permitted by law.
